Your First Visit

Welcome you to our practice. Your appointment will be scheduled so that our team can spend time with you to help answer your questions and concerns and go through your previous dental history and evaluate your treatment needs.
If possible, please bring along any previous treatment information or x-rays that you may have.

Patient History Form

At your first visit you are required to complete a Patient History Form that will be kept under strictest of confidence. This form will be kept on file and referred to at each visit.
